摘要
目的:肝细胞癌(HCC)术后早期复发会显著降低患者生活质量并缩短生存期。然而,现有模型多依赖单中心或单一维度数据,难以精准检测出HCC术后早期复发。因此,设计/评估一种可靠、无创、综合的工具来预测HCC复发风险,对指导术后个体化抗肿瘤治疗及改善预后至关重要。
患者与方法:我们回顾性纳入2012年12月至2022年12月期间在中国陆军军医大学第一附属医院接受根治性肝切除术的HCC患者(n=1424例)。按7:3比例随机分层分为训练集与测试集。利用最小绝对收缩与选择算子(LASSO)逻辑回归及多因素逻辑回归筛选最优预测因子,随后构建列线图及在线计算器。该预测模型在两所其他医疗机构(n=218例)进行外部验证。采用受试者工作特征曲线下面积(AUC)、校准曲线及决策曲线评估模型性能。
结果:列线图直观展示了HCC患者短期复发预测模型中的9个独立危险因素:Edmondson Steiner Ⅲ–Ⅳ级、肿瘤卫星结节、血管侵犯、最大肿瘤直径>5 cm、甲胎蛋白(AFP)水平≥400 μg/L、DeRitis比值≥1.49、γ-谷氨酰转移酶(GGT)水平≥63.5 U/L、预后营养指数(PNI)<46.18、中性粒细胞与淋巴细胞比值(NLR)≥1.91。训练集、测试集及验证集的AUC分别为0.760(95%置信区间:0.731–0.790)、0.784(95%置信区间:0.741–0.828)及0.787(95%置信区间:0.728–0.846),提示模型具有良好的预测性能。校准曲线与决策曲线表明,该模型可转化为切实的临床获益。
结论:我们构建并评估了一个基于炎症-免疫-营养标志物评分的列线图,用于预测HCC术后早期复发,并提供了一个免费、易用的在线计算器以便快速获取结果。该计算器帮助临床医生将复杂临床数据转化为可操作的信息,从而制定风险分层的术后管理策略。
