摘要：Bland-White-Garland 综合征（BWGS）是一种左冠状动脉异常起源于肺动脉的罕见且严重的先天性心脏病。在罕见的成年 BWGS 患者中，左冠状动脉由扩张的右冠状动脉侧支循环供血，当较高压力的冠状动脉血流流入较低压力的肺动脉导致“窃血”，严重影响心肌血流灌注。本研究报告一例 53 岁胆道结石男性患者，合并 BWGS、房颤、完全性左束支传导阻滞、慢性心力衰竭，在未经心脏手术修复冠状动脉畸形的情况下，在全身麻醉下行胆总管切开术并平稳度过围手术期。本文总结其诊治过程并回顾相关文献，归纳了 BWGS 的潜在病理生理机制和麻醉管理要点，以便对此罕见病诊治提供参考。
