摘要
背景:常染色体显性多囊肾病(ADPKD)作为最常见的遗传性肾脏疾病,常伴发高血压,且两者可能相互加剧病情。本研究采用双向孟德尔随机化(MR)设计探究PKD与高血压的因果关系,并结合生物信息学分析揭示潜在遗传机制。
方法:从国际流行病学单位(IEU)全基因组关联研究(GWAS)数据库获取PKD与高血压的遗传数据。基于与PKD及高血压高度相关的单核苷酸多态性(SNP)进行双向MR分析,并通过功能映射与注释(FUMA)平台对SNP进行遗传注释与富集分析。通过GEO数据库及单细胞数据分析工具研究PKD与对照组的基因表达差异。采用定量PCR分析正常肾小管上皮细胞(RCTEC)、肾囊性上皮细胞(WT9-12)及ADPKD肾组织中ARL13B mRNA水平。
结果:MR分析显示PKD对高血压存在因果效应(IVW:P=0.038; OR=1.011;95% CI:1.001-1.021),同时发现高血压对PKD存在反向因果效应(IVW:P=0.042;OR=1.195;95% CI:1.007-1.420)。未检测到显著异质性或多效性。基因注释识别出27个与高血压密切相关的PKD基因。其中功能富集分析表明ARL13B参与纤毛形态及功能障碍。值得注意的是,RT-PCR结果显示ARL13B mRNA在人类ADPKD肾脏(P<0.001)及WT9-12细胞(P<0.05)中显著上调。
结论:本双向MR研究证实PKD对高血压具有因果效应,且高血压反过来影响PKD进展。ADPKD中ARL13B表达升高提示纤毛相关通路可能参与肾性高血压的发生发展。
